I am loathe to give this kind of advice not knowing exactly what is causing your sciatica but I have found back extensions to be helpful. I had sciatica years ago from bulging lumbar discs. I went to a physio and he told me to lie on the floor on my stomach and to raise the top half of my body up with my arms for a few seconds before letting myself down slowly. This should be repeated several times. Of course if this causes too much pain, don't do it. I am not a physician but I have suffered similar problems. I do not feel entirely comfortable giving you this advice as I do not want to exacerbate your condition; on the other hand if this information could help you, I would not want to withhold it. Thinking of you as a reader with head forward and down more than not, maybe there could be an attempt at adjusting posture with attention to balancing the head erectly, slowly and gently, then moving this way to see if a difference is made.
